Thank you for submitting your application. There are two more steps you must complete.
1. Please pay the $25.00 application fee. You can either click the button below to pay via credit card or PayPal, or mail a check to: Service Dogs of Virginia, PO Box 408, Charlottesville, VA 22902. Your application will not be processed until we receive this fee.
2. The Medical History form must be completed by your physician and returned to Service Dogs of Virginia in order to complete your application.
